Liability Release & Assumption of Risk
I, the undersigned, acknowledge that participation in martial arts training, seminars, and events organized by the Universal Systems Martial Arts Organization (USMAO) involves inherent risks of physical injury, including but not limited to bruises, sprains, fractures, and other injuries.
In consideration of being permitted to participate in USMAO activities, I hereby voluntarily release, waive, discharge, and covenant not to sue USMAO, its officers, directors, instructors, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during or as a result of participation in any USMAO event or activity.
I represent that I am in good physical condition and have no medical condition that would prevent my safe participation. I agree to follow all safety guidelines and instructions provided by USMAO instructors and staff.
This release shall be binding upon me, my heirs, executors, administrators, and assigns.
Photo & Video Release
I grant USMAO and its authorized representatives permission to photograph and/or record video of me during USMAO events and activities. I authorize USMAO to use these images and recordings for promotional, educational, and organizational purposes, including but not limited to the USMAO website, social media, newsletters, and printed materials.
I waive any right to inspect or approve the finished product and release USMAO from any liability in connection with the use of such images or recordings. No compensation will be provided for this release.
